Canada Care Connect Inc. (CCC Inc.) offers this platform www.staffmedhomehomecare.com to individuals and families seeking to find independent third-party care providers. The care providers are not employees of CCC Inc. Services offered on the website are engaged or retained directly between the parties via the platform. All parties agree to provide and post their contact information on the site for purposes of viewing, evaluating and contacting one-another and for no other purpose. All parties acknowledge that CCC Inc. or its affiliated companies, are not responsible for the use collection or misuse of any personal information posted.

As part of the on-boarding/registration process, CCC Inc./www.staffmedhomecare.com requires all care providers to supply appropriate professional credentials including a valid and clean police records check, current identification (such as driver’s license, passport, citizenship/landed immigrant status/work permit). Care providers may also be requested from time to time by a party to supply additional information prior to being engaged by an individual or family.

Contractual Agreement for Contractors and Caregivers

It is agreed, that you’re providing your personal information for the purposes of viewing, evaluation and contacting you by prospective Clients and no other purpose.

It is also understood and agreed that you’re not an employee or contractor of Canada Care Connect Inc. or Staffmedhomecare and you’re not hired by company to perform duties. You’re acting is an independent contractor and engaging into professional relationships with Care Seekers on your own.

We can not provide you with any working hours, contracts, assignments and so on.

It is acknowledged that Canada Care Connect Inc. or its affiliated companies are not responsible for the use, collection or misuse of any personal information you’ve posted. As part of the on-boarding/registration process, Canada Care Connect/ www.staffmedhomecare.com encourages you as care provider to supply appropriate professional credentials including a valid and clean police records check, current identification (such as driver’s license, passport, citizenship/landed immigrant status/work permit). If the info is incomplete or missing, you might be contacted by prospective Client directly and you may also be requested from time to time by the interested party (Client) to supply additional information prior to being engaged by an individual or family with you.

Personal information protection policy

Canada Care Connect Inc. is committed to safeguarding the personal information entrusted to us by our Clients and caregivers. We manage your personal information in accordance with Ontario Information Protection Act and other applicable laws.

The policy outlines the principles and practices we follow in protecting your personal information.
A copy of this policy is posted as part is disclaimer and available to any Client or any caregiver.

Personal information:
Personal information is the information about an identifiable individual. This may include an individual’s name, home address, phone number, age, financial information, educational history, email address, photo, links to social media profile(s), etc.

Collection personal information:
We collect only the personal information we need for the purposes of providing services to our Clients and introduce them to available caregivers. We collect personal information directly from caregivers at the time we ask caregiver or Client to create your own account. This information is strictly confidential and kept under encrypted password created by person who create the account. Any discrepancies and not accurate submission is responsibility of employee.

Consent

We ask everyone consent to use or disclose caregiver/Client personal information to government agencies, Revenue Canada, Law enforcement offices.

We do not disclose any caregiver/Client information to any other organization or/and individual for the purposes of references, caregiver’s performance without written consent of individual who’s personal information is requested. The caregiver/Client may withdraw consent to the use, disclosure of personal information at any time, unless the personal information is necessary for us to fulfill our legal obligations.

Caregiver/Client may cancel his/her account at any time without prior notice to us.
We may not request consent when the collection, use, or disclosure is to determine suitability for an honor or award, or in an emergency that threatens life, health or safety.

If we wish to use or disclose your personal information for any new business purpose, we will ask for consent. We may not seek consent if the law allows this (e.g the las allows organizations to use personal information without consent for the purpose of collection a debt).
Personal information being collected, use and disclosed

We collect, use and disclose personal caregiver information to meet the following purposes:
-Determine eligibility for providing services incl. verifying qualification and references
-Establishing training and development requirements
-Assessing performance and managing performance issues if they arise
-Administering pay
-Complying with applicable laws (Canada income TAX Act, Industry Standards)

We only collect amount of personal information that is reasonable to meet above purposes.
-Contact information such as your name, home address, telephone number
-Criminal backgrounds check
-Employment information such as education, background, work history an references, reference information and interview notes, letter of offers an acceptance of employment, policy acknowledgement forms, background verification information, workplace performance evaluation, emergency contacts, etc.
-Financial information such bank, transit and account in order to deposit payment for services via e-commerce
-Other personal information required of the purposes of providing caregiver services.
We do not disclosed your personal information
We do not disclose your personal information to any organization or individuals other that Law enforcement agencies, police, in order to help in crime investigations, Revenue Canada for tax purposes, government agencies. We do not disclose your information for references any kind, we use 2 ways online rating system.

Safeguard and secure personal information

We make every reasonable effort to unsure that personal information is accurate and complete. We rely on individuals to notify us if there is a change to their personal information that may affect their relationship with our organization. If you are aware of an error in our information about you, please let us know and we will correct it. In some cases we might ask the written request for correction or submit correction in writing.

We protect personal information in a manner appropriate for the sensitivity of the information. We make every reasonable effort to prevent any loss, misuse, disclosure or modification of personal information, as well as any authorized access to personal information.
We do not print out hard copies of caregivers or Clients’ files. The electronic files are secured by encrypted password and accessible only by System Administrator. All electronic records are securely stored on server can be accessed by network administrator.

Safeguard other important information

Sometimes in our communication with Clients we use an email for business related information regarding caregivers, work hours, cancellations and other administrative duties.
Normally email communication is secure and emails are addressed to particular Client who requested certain information. In case electronic mail was sent to the wrong recipient we ask to delete the email email immediately and notify us about this error.

We use attached privacy statement at the bottom of email stating that any distribution our email is strictly prohibited.

We use appropriate security measures when destroying personal and other information, including shredding paper records and permanently deleting electronic records.
We keep inactive employees’ personal information on file 6 months and the files are shredded. We retain personal information as long is reasonable to fullfil the purposes for which the information was collected or for legal or business purposes.

Access to personal/Client information

We grant you access to your file in case you need to make correction, change or add your personal information.

We refuse to provide with personal information of our caregiver another caregiver and/or Client in order to contact the person and/or discuss issues. We decline request of Client to reveal employee phone number in order to clarify work related information, however caregiver might allow Client to access caregiver’s professional profile in order to familiarize themselves with caregiver’s skills and background as well as to check caregiver’s police records check once selecting to provide services

This policy was customized based of nature of our business, Ontario Personal Information Protection Act and confidentiality statement.
